In the event I am approved for in-store credit I hereby authorize and request to initiate credit entries, and if necessary, debt entries and adjustments for any credit entries in error, to this account at the financial institution named above. This authorization is to remain in effect until withdrawn in writing. I further certify that I am authorized to execute this agreement on behalf of the above named account holder.I (we) hereby authorize FastCredit$2K to check my credit, contact my personal references or any otherperson at anytime to verify any information I have given to aid in the approval and/or collection of this account. I am aware that this does not mean that I am approved for in-store credit until my credit is reviewed by store credit department.
